Application for graduation
A complete graduation application consists of:
  • A Bachelor Degree Application Form with advisor signature. A separate application should be filed for each degree to be received (e.g., BA, BS, BM, BSN, BFA). [Alternative link to bachelor degree application.]
  • A completed major worksheet. If you are pursuing a double major, you must submit a worksheet for each major.
  • A completed minor worksheet, if applicable, or a list of courses that will be used to satisfy minor requirements for each of your minors.
  • Application for Major in Accounting or Business Administration
Once you have completed the required pre-business courses (see Degree Requirements), you must apply to be a business or accounting major. Most students apply during their sophomore year.  This is the form to use.

Application for Minor in Business Administration
If you want to pursue a different major, but also want some documented exposure to business and accounting, you may use this form to apply for the business minor.

  • The form must be approved by the advisor, division head of the major, and the Registrar. The Pre-Approval of Transfer Credit form should be submitted for approval prior to enrolling in courses at another institution.
